Job summary

Advocate Health is seeking a hospital-based physical therapist (PT) with a DPT/PT degree and active state licensure. CPR/BLS certification is required, and prior hospital experience is preferred. You will document progress notes, maintain detailed patient records, and participate in quality initiatives while supporting a safe, productive hospital workflow.

Qualifications

• B.S., M.S., or D.P.T. in physical therapy from an accredited school.
• Current licensure in the applicable state and CPR/BLS Level C.
• Previous hospital experience preferred.

Responsibilities

• Assess and treat patients with comprehensive evaluations and goals.
• Document evaluations, progress notes, and discharge notes per policy.
• Maintain accurate and timely patient records.
• Supervise support staff assisting with patient care.
• Participate in process improvement and quality initiatives.
• Communicate effectively with patients, families and staff.
• Promote professional development and education of healthcare workers.
