Job summary

Advocate Health is seeking a Physical Therapist to deliver age-appropriate patient care, evaluations, and treatment planning in a hospital or clinic setting. The clinician will collaborate with a multidisciplinary team, educate patients and families, and ensure compliant documentation and ongoing professional development.

The role requires a PT license in the applicable states, BLS certification, and ongoing CEU participation. Flexibility to work across departments is expected.

Qualifications

• Degree in Physical Therapy from an accredited institution.
• Doctor of Physical Therapy (DPT) preferred or educational requirements commensurate with the date of graduation.
• Physical Therapy License in the state(s) of employment.
• BLS certification for Healthcare Providers must be obtained and maintained.
• Continuing education requirements must be maintained as mandated by state professional standards.

Responsibilities

• Age-appropriate patient care with age-specific needs and policies.
• Perform evaluations and develop patient care plans with evidence-based interventions.
• Set functional goals and monitor outcomes; coordinate with the care team.
• Educate patients and caregivers; provide home instruction and discharge planning.
• Maintain professional documentation, billing, and compliance; adhere to HIPAA.
• Contribute to department operations and participate in professional development.
• Adapt to different departments/locations as needed under Enterprise Float Program.

Education

Doctor of Physical Therapy (DPT) preferred
