Job summary

Advocate Health’s High Point Medical Center is seeking a full‑time Physical Therapist for the Outpatient Rehabilitation: Elm location. You will evaluate and treat patients, develop plans of care, and document progress per policy. Relocation assistance is available for qualified candidates.

The role requires a B.S./M.S./D.P.T. from an accredited PT program, current state licensure, and CPR/BLS Level C certification.

Qualifications

• B.S., M.S., or D.P.T degree from accredited physical therapy school.
• Current licensure in the state of applicable state
• CPR certification in Basic Life Support, Level C.

Responsibilities

• Adheres to the general hospital standards to promote a cooperative work environment by utilizing communication skills, interpersonal relationships and team building.
• Provides patient care assessment and treatment. Ensures patient evaluations and treatments are comprehensive, functional and according to the Standard of Practice.
• Maintains accurate documentation. Completes written patient evaluations, progress notes, and discharge notes per policy.
• Ensures that documentation is legible.
• Meets department financial and productivity standards. Understands the budgeted expectations.
• Participates in process management to optimize quality and efficiency of services.
• Promotes professional development and education of other health care workers and provides representation on committees.
• Communicates effectively. Communicates in a professional and timely manner with patients, physicians, staff, and all external customers.
• Provides appropriate patient care in accordance with age/developmental guidelines.
