Job summary

Advocate Health seeks a licensed Physical Therapist to manage a home health caseload, delivering prescribed therapies and coordinating a multidisciplinary team. You will document care, monitor patient progress, and adjust plans with physician input.

The role requires a DPT, state PT licensure, and a valid driver’s license with reliable auto insurance; BLS certification is also required. Travel across assigned territories is expected, in a dynamic home health environment.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Physical Therapy License in the state where you work.
• Current state driver's license and auto insurance at all times.
• Basic Life Support for Healthcare Provider certification from the American Heart Association required.

Responsibilities

• Manage daily patient caseload and ensure timely care and documentation.
• Analyze data from assessments to determine issues and resources.
• Identify high-risk patients and coordinate care to mitigate risk.
• Evaluate PT patients to determine rehabilitation needs and plan of care.
• Direct care coordination of multidisciplinary team and supervise assistants.
• Maintain communication with physician and branch supervision.
• Perform treatments in accordance with regulatory guidelines.
