Job summary

Advocate Health is hiring a licensed Physical Therapist to join our team. You will evaluate and treat patients, document evaluations and progress, ensure accurate coding and scheduling, and work toward department productivity goals while supporting staff and student learning.

You will collaborate with physicians and care teams, maintain compliance with standards, and help advance quality and safety initiatives across care settings.

Qualifications

• Bachelor of Science degree from an accredited physical therapy school required.
• Licensed to practice Physical Therapy in the applicable state with current licensure.
• CPR certification required every two years.

Responsibilities

• Provides patient care assessment and treatment.
• Maintains accurate patient documentation, including evaluations, progress notes and discharge notes.
• Ensures documentation of education, charges and statistics with ICD-9 codes as appropriate.
• Meets department productivity standards and participates in process improvement.
• Supervises students and provides in-service training to staff.
• Communicates professionally with patients, physicians, staff and external customers.
