In this role, you will evaluate and treat residents, develop individualized treatment plans, collaborate with the interdisciplinary care team, and help residents achieve their highest level of mobility, function, and independence.Key ResponsibilitiesEvaluate residents within 24 hours, or one business day, of physician referral whenever possibleDevelop individualized treatment plans and obtain physician approval as requiredProvide physical therapy treatments in accordance with physician orders and established goalsMonitor resident progress and modify treatment plans as appropriateAssist the nursing department with training Restorative AidesSupervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state practice regulationsCommunicate effectively with physicians, nursing staff, residents, families, and interdisciplinary team membersParticipate in care conferences, utilization review meetings, rehabilitation meetings, and discharge planning activitiesComplete evaluations, treatment notes, progress notes, recertifications, and other required documentation accurately and timelyEducate residents, families, and caregivers on therapy goals, home exercise programs, safety techniques, and discharge recommendationsRecommend and facilitate acquisition of durable medical equipment such as walkers, canes, braces, grab bars, and elevated toilet seatsReport equipment concerns and assist in maintaining a safe therapy environmentConduct in-service education and training programs for facility staff as neededSupervisory ResponsibilitiesSupervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state regulations and facility policiesQualificationsEducation & ExperienceBachelor's degree in Physical Therapy requiredMaster's or Doctorate degree in Physical Therapy preferredExperience in skilled nursing, rehabilitation, or long-term care preferredExperience with PointClickCare (PCC) and Casamba preferredStrong clinical, communication, and documentation skillsLicenses & CertificationsCurrent South Carolina Physical Therapist license in good standingMust maintain all required continuing education requirementsMust remain in good standing with the applicable State BoardWhat Makes You a Great FitPassion for helping residents regain mobility and independenceStrong clinical judgment and problem-solving skillsExcellent communication and interpersonal abilitiesDependable and flexibleTeam-oriented and resident-focusedCommitment to delivering exceptional therapy outcomesWe are an equal opportunity employer.
