Job summary

West Village Post Acute in Greenville, SC is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team on a PRN basis. You will evaluate and treat residents, develop individualized treatment plans, and collaborate with an interdisciplinary team to help residents achieve their highest level of mobility, function, and independence.

This PRN role offers competitive pay ($40–$50/hour), flexible scheduling with evenings/weekends, and advancement opportunities within our Upstate

Qualifications

• Bachelor's degree in Physical Therapy required.
• Master's or Doctorate in Physical Therapy preferred.
• Experience in skilled nursing or long-term care preferred.
• Experience with PCC and Casamba preferred.
• Current SC Physical Therapist license in good standing.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy to improve mobility and independence.
• Monitor progress and adjust treatment plans.
• Document accurately per regulations.
• Educate residents and families on therapy goals and home programs.
• Participate in care planning and discharge planning.
• Supervise PTAs and therapy aides as allowed.
• Ensure compliance with state practice acts and policies.
