Job summary

River Falls Post Acute in Marietta, South Carolina, is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. Under the supervision of a licensed Physical Therapist, you will provide skilled therapy focused on mobility, strength, and independence to residents.

Qualifications

• Graduate of an accredited PTA program.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with PCC and Casamba preferred.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Provide physical therapy treatments as directed by the supervising Physical Therapist.
• Follow physician orders and individualized treatment plans.
• Document daily treatments, weekly progress notes, and resident outcomes accurately and timely.
• Participate in patient care conferences and rehabilitation meetings as needed.
• Monitor resident progress and communicate updates to the Physical Therapist.
• Assist with cleaning and maintenance of therapy equipment and treatment areas.
• Train and assist Restorative Aides in carrying out therapy-related programs.
• Communicate effectively with residents, families, nursing staff, and interdisciplinary team members.
• Participate in staff education and in-service training programs.
• Educate residents, families, and caregivers on maintenance programs and discharge recommendations.
• Report equipment concerns and assist in maintaining a safe therapy environment.
• Ensure compliance with all facility policies, state regulations, and payer requirements.

• Experience with PointClickCare (PCC) and Casamba preferred
• Strong communication, documentation, and organizational skills

Licenses & Certifications

• Current South Carolina Physical Therapist Assistant (PTA) license in good standing
• Must maintain all required continuing education requirements
• Must remain in good standing with the applicable State Board
