Chandler Creek Post Acute in Greer, SC is seeking a Physical Therapist (PT) on a PRN basis. You will evaluate residents, design personalized therapy plans, and help patients regain mobility and independence in a supportive rehab team.

Competitive pay $45-$50 per hour with flexible PRN scheduling. Prior long-term care or rehab experience and SC PT license required. Join the largest post-acute network in SC and make a difference in residents' quality of life.

Qualifications

• Bachelor's degree required; advanced degree preferred.
• Active, unencumbered Physical Therapist license in South Carolina.
• Experience in long-term care or rehab setting preferred.
• Familiarity with PCC and Casamba preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Deliver interventions to improve strength, balance, and mobility.
• Monitor progress and adjust treatment plans as needed.
• Maintain accurate and timely documentation.
• Collaborate with interdisciplinary teams to support outcomes.
• Educate residents and families on therapy techniques and goals.

Position Summary
