Job summary

River Falls Post Acute in Marietta, South Carolina is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team on a PRN basis. You will evaluate and treat residents, develop individualized treatment plans, collaborate with the interdisciplinary care team, and help residents achieve their highest mobility, function, and independence.

This PRN opportunity offers flexible scheduling, competitive pay of $45–$50 per hour, and a supportive leadership team.

Qualifications

  • Bachelor's degree in Physical Therapy required.
  • Master's or Doctorate degree in Physical Therapy preferred.
  • Experience in skilled nursing, rehabilitation, or long-term care preferred.
  • Experience with PCC and Casamba preferred.
  • Strong clinical, communication, and documentation skills.

Responsibilities

  • Evaluate residents within 24 hours, or one business day, of physician referral whenever possible.
  • Develop individualized treatment plans and obtain physician approval as required.
  • Provide physical therapy treatments in accordance with physician orders and established goals.
  • Monitor resident progress and modify treatment plans as appropriate.
  • Assist the nursing department with training Restorative Aides.
  • Supervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state practice regulations.
  • Communicate effectively with physicians, nursing staff, residents, families, and interdisciplinary team members.
  • Participate in care conferences, utilization review meetings, rehabilitation meetings, and discharge planning activities.
  • Complete evaluations, treatment notes, progress notes, recertifications, and other required documentation accurately and timely.
  • Educate residents, families, and caregivers on therapy goals, home exercise programs, safety techniques, and discharge recommendations.
  • Recommend and facilitate acquisition of durable medical equipment such as walkers, canes, braces, grab bars, and elevated toilet seats.
  • Report equipment concerns and assist in maintaining a safe therapy environment.
  • Conduct in-service education and training programs for facility staff as needed.

Position Summary

Qualifications
Education & Experience
  • Bachelor's degree in Physical Therapy required
  • Master's or Doctorate degree in Physical Therapy preferred
  • Experience in skilled nursing, rehabilitation, or long-term care preferred
  • Experience with PointClickCare (PCC) and Casamba preferred
  • Strong clinical, communication, and documentation skills
Licenses & Certifications
  • Current South Carolina Physical Therapist license in good standing
  • Must maintain all required continuing education requirements
  • Must remain in good standing with the applicable State Board
