In this role, you will evaluate and treat residents, develop individualized treatment plans, and collaborate with an interdisciplinary team to help residents achieve their highest level of mobility, function, and independence.## Key Responsibilities* Evaluate residents and develop individualized treatment plans based on physician orders and clinical needs* Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence* Monitor resident progress and modify treatment plans as appropriate* Communicate with physicians, nursing staff, residents, families, and interdisciplinary team members regarding treatment goals and outcomes* Complete accurate and timely documentation in accordance with facility, state, federal, and payer requirements* Participate in care planning meetings, rehabilitation conferences, and discharge planning activities* Educate residents, families, and caregivers on therapy goals, safety techniques, and home exercise programs* Conduct in-service education and training programs for facility staff as needed* Ensure compliance with all state practice acts, facility policies, and regulatory requirements* Promote resident safety, independence, and quality of life## ## Supervisory Responsibilities* Supervise Physical Therapist Assistants (PTAs), therapy aides, and students in accordance with state regulations and facility policies## ## Qualifications### Education & Experience* Bachelor's degree in Physical Therapy required* Master's or Doctorate degree in Physical Therapy preferred* Experience in skilled nursing, rehabilitation, or long-term care preferred* Experience with **PointClickCare (PCC)** and **Casamba** preferred* Strong communication, documentation, and clinical skills### Licenses & Certifications* Current **Physical Therapist license in South Carolina** in good standing* Must maintain all required continuing education requirements* Must remain in good standing with the applicable State Board## ## What Makes You a Great Fit* Passion for helping residents improve mobility and independence* Strong clinical judgment and problem-solving abilities* Excellent communication and interpersonal skills* Dependable and flexible* Team-oriented mindset with a resident-centered approach* Commitment to delivering exceptional care and outcomes##We are an equal opportunity employer.
