River Falls Post Acute in Marietta, SC is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. Under the direction of a licensed Physical Therapist, the PTA will provide skilled therapy services to help residents improve mobility, strength, and independence while enhancing quality of life.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with PointClickCare (PCC) and Casamba preferred.
• Strong communication, documentation, and organizational skills.

Responsabilités

• Provide physical therapy treatments as directed by the supervising Physical Therapist (PT).
• Follow physician orders and individualized treatment plans.
• Document daily treatments, weekly progress notes, and resident outcomes accurately and timely.
• Participate in patient care conferences and rehabilitation meetings as needed.
• Monitor resident progress and communicate updates to the Physical Therapist.
• Educate residents, families, and caregivers on maintenance programs and discharge recommendations.

Position Summary

We are seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. Under the direction of a licensed Physical Therapist, the PTA will provide skilled therapy services designed to help residents improve mobility, strength, function, and independence while enhancing their quality of life.

Key Responsibilities

• Provide physical therapy treatments as directed by the supervising Physical Therapist
• Follow physician orders and individualized treatment plans
• Document daily treatments, weekly progress notes, and resident outcomes accurately and timely
• Participate in patient care conferences and rehabilitation meetings as needed
• Monitor resident progress and communicate updates to the Physical Therapist
• Assist with cleaning and maintenance of therapy equipment and treatment areas
• Train and assist Restorative Aides in carrying out therapy-related programs
• Communicate effectively with residents, families, nursing staff, and interdisciplinary team members
• Participate in staff education and in-service training programs
• Educate residents, families, and caregivers on maintenance programs and discharge recommendations
• Report equipment concerns and assist in maintaining a safe therapy environment
• Ensure compliance with all facility policies, state regulations, and payer requirements

Education & Experience

• Graduate of an accredited Physical Therapist Assistant program
• Experience in skilled nursing, rehabilitation, or long-term care preferred
• Experience with PointClickCare (PCC) and Casamba preferred
• Strong communication, documentation, and organizational skills

Licenses & Certifications

• Current South Carolina Physical Therapist Assistant (PTA) license in good standing
• Must maintain all required continuing education requirements
• Must remain in good standing with the applicable State Board
