Job summary

San Francisco Post Acute is seeking a PRN Physical Therapist to join our geriatric and post-acute care team in San Francisco, CA. The role offers flexible scheduling and competitive compensation, ideal for therapists seeking supplemental work or flexible hours.

The PT will assess patients, create individualized plans, and collaborate with a multidisciplinary team to improve mobility and function. A California PT license and current BLS/CPR are required.

Qualifications

• Bachelor's, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
• Active, unrestricted Physical Therapist license in California.
• Current BLS/CPR certification.
• Experience in SNF/acute/post-acute settings is preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans with functional goals.
• Administer evidence-based PT interventions to improve mobility, strength, balance, and coordination.
• Document progress, updates, and discharge plans per facility and regulatory standards.
• Collaborate with physicians, OT, SLP, and nursing staff.
• Educate patients and families on home exercise programs and safe mobility.

Position Overview

We are seeking a PRN Physical Therapist who is passionate about delivering exceptional geriatric and post-acute care. Key Responsibilities

• Evaluate patients, develop individualized treatment plans, and establish realistic, functional goals.
• Administer evidence-based physical therapy interventions to improve mobility, strength, balance, and coordination.
• Document patient progress, updates, and discharge plans accurately and timely in compliance with facility and regulatory standards.
• Collaborate closely with a multidisciplinary team, including physicians, occupational therapists, speech-language pathologists, and nursing staff.
• Educate patients, families, and caregivers on home exercise programs and safe mobility techniques.

Qualifications

• Minimum of a Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Active, unrestricted Physical Therapist license in the State of California.
• Current BLS/CPR certification.
• Previous experience in a skilled nursing facility (SNF), acute care, or post-acute rehab setting is preferred, but open to all qualified applicants.
• Strong communication, organizational, and interpersonal skills.
