Job summary

Peninsula Post Acute in Burlingame, CA is seeking a compassionate Physical Therapist (PT) for our in-house rehab team on a PRN basis. This role offers flexible scheduling while working in a supportive, patient-centered environment focused on clinical excellence.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide evidence-based physical therapy services to improve mobility, strength, balance, endurance, and overall functional independence.
• Monitor resident progress and modify treatment plans as appropriate to achieve optimal outcomes.
• Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
• Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to coordinate high-quality patient care.
• Supervise Physical Therapist Assistants (PTAs) in accordance with California practice guidelines.
• Participate in interdisciplinary care conferences and discharge planning.
• Educate residents and caregivers on home exercise programs, fall prevention strategies, and safe mobility techniques.
