Santa Cruz Post Acute in Santa Cruz, CA, is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. We offer full-time, part-time, and PRN positions with a starting rate of $50–$58 per hour DOE plus benefits.

The PT will evaluate and plan treatment to improve strength, mobility, and independence, collaborating with the care team to deliver patient-centered care in a supportive setting. New graduates are welcome to apply.

Qualifications

• Licensed Physical Therapist in California.
• Graduate of an accredited Physical Therapy program.
• Strong communication and documentation abilities.
• SNF experience preferred but not required; new graduates welcome.

Responsibilities

• Evaluate and plan physical therapy services to restore strength, mobility, balance and functional independence.
• Provide therapeutic interventions including gait training, strengthening, balance activities, pain management, and mobility training.
• Document evaluations, daily notes, progress reports, and discharge summaries per standards.
• Collaborate with nursing, therapy, physicians, and families to support resident goals.
• Educate residents and caregivers on exercise programs and safety techniques.
• Participate in interdisciplinary care conferences and contribute to care planning.
• Maintain compliance with state, federal, and professional regulations.

Santa Cruz Post Acute is seeking a dedicated and compassionate Physical Therapist (PT) to join our skilled rehabilitation team. Position Summary

The Physical Therapist is responsible for evaluating, planning, and providing skilled physical therapy services to help residents restore strength, mobility, balance, and functional independence. This role works closely with the interdisciplinary care team to ensure high‑quality outcomes.
