Job summary

San Francisco Post Acute in San Francisco, CA, seeks a PRN Physical Therapist to join our therapy team on a per diem basis. The role offers competitive pay and a highly flexible schedule to accommodate personal commitments while delivering high-quality geriatric post-acute care.

Responsibilities include evaluating patients, creating treatment plans, documenting progress, and educating patients and families on home programs. A CA PT license is required, with CPR/BLS current.

Qualifications

• Active, unrestricted Physical Therapist license in California.
• Strong communication, organizational, and interpersonal skills.
• BLS/CPR certification current.

Responsibilities

• Evaluate patients, develop individualized treatment plans and goals.
• Administer evidence-based PT to improve mobility, strength, balance and coordination.
• Document progress and discharge plans per facility and regulatory standards.
• Collaborate with physicians, OTs, SLPs and nursing staff.
• Educate patients and families on home exercise programs and safe mobility.
