Central Gardens Post Acute in San Francisco, CA is seeking a compassionate Physical Therapist to join our in-house rehabilitation team. We offer Full-Time, Part-Time and PRN opportunities with flexible scheduling designed to support your professional and personal needs.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled physical therapy services based on residents' needs and goals.
• Implement evidence-based interventions to improve strength, mobility, balance, endurance, and functional independence.
• Monitor resident progress and adjust treatment plans as appropriate.
• Educate residents and families regarding therapy goals, exercises, mobility, and safety.
• Collaborate with PTAs and the interdisciplinary care team to support positive clinical outcomes.
• Maintain accurate and timely clinical documentation.
• Promote resident safety, dignity, independence, and quality of life.
• Participate in care planning and interdisciplinary team activities as needed.
