Job summary

San Francisco Post Acute in San Francisco, CA seeks a PRN Physical Therapist to join our geriatric and post-acute care team. This role offers flexible scheduling and competitive pay, ideal for therapists enhancing income or balancing commitments.

Responsibilities include evaluating patients, developing treatment plans, documenting progress, and collaborating with physicians, therapists, and nursing staff. Licensed PT in CA with BLS/CPR and experience in SNF or post-acute rehab is preferred.

Qualifications

• Active, unrestricted Physical Therapist license in California.
• Current BLS/CPR certification.
• Experience in SNF/acute/post-acute rehab preferred.

Responsibilities

• Evaluate patients, develop individualized treatment plans, and set functional goals.
• Administer PT interventions to improve mobility, strength, balance, and coordination.
• Document progress and discharge plans per regulatory standards.
• Collaborate with physicians, therapists, and nursing staff.
• Educate patients and families on home exercise programs and safety.
