Job description

Job Title: Physical Therapist (PT)

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time opportunity performing home health visits in our Person County office, located in Roxboro, North Carolina. This role involves servicing adult and geriatric clients across territories in Caswell County.

As a home care Physical Therapist, you will be a key member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care while remaining at home.

Responsibilities:

1. Make home visits to clients within designated territories.
2. Perform diagnostic tests such as mobility, joint range of motion, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Develop and implement individualized care plans including therapeutic exercises, gait training, and balance interventions.
4. Assess and revise care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
6. Document observations and interventions accurately using electronic medical records.
7. Implement wound care interventions using evidence-based dressings and topical agents.

Qualifications:

1. Current North Carolina Physical Therapist (PT) license.
2. Graduation from an APTA-approved program or equivalent.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; EMR experience preferred.
