Job Title: Physical Therapist (PT) - Per Diem

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per diem opportunity providing home health visits in our Person County office located in Roxboro, North Carolina. This role involves serving adult and geriatric clients across Granville and Vance counties.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary team offering rehabilitative care and skilled nursing, enabling clients to remain at home with appropriate medical support.

Responsibilities:

1. Conduct home visits to clients within designated territories.
2. Perform diagnostic tests such as mobility assessment, strength, balance, and safety evaluations.
3. Develop and implement individualized care plans including manual exercises, gait training, and balance interventions.
4. Assess and update care plans regularly and collaborate with nursing staff in multidisciplinary planning.
5. Educate clients and their families on rehabilitative activities to promote health and safety.
6. Document all observations and interventions accurately using electronic medical records.
7. Implement wound care interventions using evidence-based dressings and topical agents.

Qualifications:

1. Current North Carolina Physical Therapist (PT) license.
2. Graduation from an APTA-approved program or equivalent.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency in computer skills; EMR experience preferred.
