Job description

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a PER DIEM home health position in our Person County Visits office in Roxboro, North Carolina. This role involves providing services to adult and geriatric clients across Granville and Vance counties.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary team delivering rehabilitative and skilled nursing care to help clients remain at home.

Responsibilities:

1. Make home visits within designated territories.
2. Perform diagnostic tests and measurements, including mobility, joint range, stability, ambulation, muscle strength, endurance, balance, and safety assessments.
3. Develop and update individualized care plans with interventions like manual therapy, gait training, and balance exercises.
4. Assess and revise care plans regularly, collaborating with nursing staff as needed.
5. Educate clients and families on rehabilitative activities to promote health, safety, and independence.
6. Document observations and interventions accurately using electronic tablets.
7. Implement wound prevention and repair interventions using evidence-based dressings and topical agents.

Qualifications:

• Current North Carolina Physical Therapist license.
• Graduation from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with EMR preferred.
