Job Description: Physical Therapist (PT) at BAYADA Home Health Care

BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time opportunity providing home health visits in our Guilford County office, located in Greensboro, NC.

This role involves delivering home health services to adult and geriatric clients across territories in Rockingham or Guilford counties. As a vital part of a multidisciplinary team, you will offer rehabilitative care and skilled nursing to enable clients to remain at home.

Responsibilities:

1. Make home visits to clients in designated geographic areas.
2. Conduct diagnostic tests and measurements related to mobility, strength, balance, and safety.
3. Implement wound care interventions using evidence-based dressings and topical agents.
4. Develop and execute individualized care plans, including manual therapy, gait training, and balance interventions.
5. Assess and update care plans regularly, collaborating with nursing staff as needed.
6. Educate clients and families on rehabilitative activities to promote health and independence.
7. Document observations and interventions accurately using electronic tablets.

Qualifications:

1. Current North Carolina Physical Therapist (PT) license.
2. At least one year of recent PT experience.
3. Graduation from an accredited program by the American Physical Therapy Association or equivalent.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal and organizational skills.
6. Proficiency with computers; experience with EMR preferred.
