Job Purpose

To provide evaluative, consultative, and treatment interventions to all home health patients in their place of residence referred by a licensed physician, maintaining effective communication with patients, families, and healthcare partners.

Key Responsibilities

• Evaluate the patient’s level of function and skilled therapy needs.
• Treat the patient to relieve pain, develop or restore function, and obtain maximum outcomes according to the plan of care.
• Advise and consult with family and interdisciplinary team members to promote optimal outcomes.
• Assess, monitor, document, and report patient's health progress using required documentation and technology within established timelines.
• Collaborate with the team and physicians to develop and manage the patient's care plan, making referrals as needed.
• Demonstrate competency in home care and specific patient care skills.
• Conduct field visits to evaluate, initiate care, monitor outcomes, and participate in quality improvement initiatives.
• Use accepted teaching materials for patient and family education.
• Complete timely and accurate documentation, including assessments, care plans, and physician orders.
• Meet productivity standards and utilize resources efficiently.
• Participate in surveys, in-services, meetings, and improvement programs.
• Follow safety procedures and report hazards.
• Maintain current licensure, certifications, and records per regulations.
• Prioritize patient care needs and demonstrate initiative and judgment.
• Adapt to workload changes and assist in training others.
• Report any violations or concerns to supervisors.
• Manage time effectively to meet goals and deadlines.

Licensure, Certification, and Education Requirements

• Valid professional license in the practicing state.
• Graduate of an accredited Physical Therapy Program.
• Foreign-trained applicants must have credentials deemed equivalent to U.S. standards by an approved evaluation organization.
