Job summary

Pride Health is seeking a licensed Physical Therapist to provide skilled home health PT in Wallingford, CT. The role covers Milford and Fairfield area patients, with a local 14-week contract and hourly pay of $50–$57.

Responsibilities include comprehensive evaluations, personalized treatment plans, and ongoing reassessments to improve function. A valid CT PT license and reliable transportation are required.

Schedule: Monday-Friday, approximately 8:30 AM - 4:30 PM

Location: Wallingford, CT 06492

Float / Coverage Area: 1) Milford CT and surrounding towns 2) Fairfield CT and surrounding towns

Job Type: Local contract

Contract Length: 14 weeks Pay Range: $50.00 to $57.00/hour*

Mileage Reimbursement Rate (if applicable): $0.52/ mile

Duties

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on patient needs and physician orders.
• Provide skilled home care physical therapy services focused on improving strength, balance, mobility, endurance, safety, and functional independence.
• Conduct ongoing patient assessments and modify treatment plans as appropriate to achieve optimal outcomes.
• Educate patients, families, and caregivers on therapeutic exercises, fall prevention, mobility techniques, safety strategies, and home exercise programs.
• Complete accurate and timely clinical documentation in accordance with agency, state, and federal guidelines.
• Collaborate closely with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary team members to coordinate patient care.
• Maintain productivity standards while delivering exceptional patient-centered care.
• Participate in care conferences and contribute to individualized patient care planning.

Requirements

• Active Physical Therapist (PT) License in the State of Connecticut.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment and treatment planning skills.
• Excellent communication, organization, and time-management abilities.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently in a home health environment.
