Job summary

Pride Health is seeking a Physical Therapist (Home Health) to support patient care in Easton, MD. This local contract position runs Monday–Friday, 8:00 AM–4:30 PM, with a 22+ week duration and potential extension. You will travel within Queen Annes County and Northern Caroline County, providing PT evaluations, treatment, and caregiver education.

Licensed PT with CPR certification is required. Minimum 1 year home health experience; HCHB and OASIS experience a plus.

Qualifications

• Active Physical Therapist license (PT) required.
• CPR Certification required.
• At least 1 year of home health experience; experience with HCHB & OASIS is a plus.

Responsibilities

• Perform comprehensive PT evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks.
• Develop and adjust individualized care plans to improve mobility, strength, balance, endurance, and independence.
• Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, and home safety education.
• Educate patients and caregivers on home exercise programs, fall prevention, and energy conservation techniques.
• Complete accurate documentation (evaluations, visit notes, progress reports, discharge summaries) using EMR systems.
• Coordinate care with physicians and allied health team members, including nurses and other therapists.
• Communicate changes in patient condition or barriers promptly to the care team.
• Participate in case conferences and care coordination activities.
• Ensure CMS regulations and infection control standards are followed; adhere to agency policies.
• Manage a mobile caseload efficiently while meeting productivity and documentation expectations.
• Adapt quickly to new environments, workflows, and patient populations as a travel clinician.

Job Details

Schedule: Monday-Friday | 8:00 AM-4:30 PM

Coverage Area: Queen Annes County, Northern Caroline County

Contract Length: 22+ Weeks Contract (Possibility to Extend)

Job Type: Local Contract

Pay Range: $56.00 - $60.00/hour (W-2) Plus Mileage reimbursement

Mileage Reimbursement Rate: $0.52/ mile

*Pay offered is based on experience, expertise, credentialing, and education

Duties

• Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks.
• Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence.
• Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education.
• Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques.
• Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems.
• Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides.
• Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team.
• Participate in case conferences, care coordination activities, and team collaboration.
• Ensure compliance with CMS regulations, infection control standards, and agency policies.
• Manage a mobile caseload efficiently while meeting productivity and documentation expectations.
• Adapt quickly to new environments, workflows, and patient populations as a travel clinician.

Requirements

• License(s): Active Physical Therapist license, CPR Certification
• Years of experience: At least 1 year of home health experience and experience with HomeCare HomeBase (HCHB) & OASIS is a plus
