Job summary

Pride Health is seeking a Physical Therapist (PT) - Home Health to support our client in Wallingford, CT. You will perform comprehensive PT evaluations and develop individualized plans of care for home health patients.

The schedule is Monday–Friday, about 8:30 AM–4:30 PM, for a local 14-week contract. Pay ranges from $50.00 to $57.00 per hour with mileage reimbursement where applicable; CT license and PT program graduation are required.

Qualifications

Responsibilities

• Perform PT evaluations and develop treatment plans based on orders.
• Provide home health PT services to improve strength, balance, and mobility.

• Document accurately per guidelines.
• Collaborate with physicians and therapists to coordinate care.

Pride Health is hiring a Physical Therapist (PT) - Home Health to support our client's healthcare facility in Wallingford, CT.

Job Details:

• Schedule: Monday-Friday, approximately 8:30 AM - 4:30 PM
• Location: Wallingford, CT 06492
• Float / Coverage Area: Middletown, CT and surrounding towns
• Job Type: Local contract
• Contract Length: 14 Weeks
• Pay Range: $50.00 to $57.00/hour*
• Mileage Reimbursement Rate (if applicable): $0.52/ mile

*Pay offered is based on experience,expertise, credentialing, and education.

Duties:

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on patient needs and physician orders.
• Provide skilled home care physical therapy services focused on improving strength, balance, mobility, endurance, safety, and functional independence.
• Conduct ongoing patient assessments and modify treatment plans as appropriate to achieve optimal outcomes.
• Educate patients, families, and caregivers on therapeutic exercises, fall prevention, mobility techniques, safety strategies, and home exercise programs.
• Complete accurate and timely clinical documentation in accordance with agency, state, and federal guidelines.
• Collaborate closely with physicians, nurses, occupational therapists, speech therapists, and other interdisciplinary team members to coordinate patient care.
• Maintain productivity standards while delivering exceptional patient-centered care.
• Participate in care conferences and contribute to individualized patient care planning.

Requirements:

• Active Physical Therapist (PT) License in the State of Connecticut.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment and treatment planning skills.
• Excellent communication, organization, and time-management abilities.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently in a home health environment.
