Job summary

Pride Health is actively hiring a Physical Therapist to join a Home Health Program serving Dallas, TX. The role features a 40-hour workweek, Monday–Friday, with a 6+ month duration and a pay rate of $55.00/hour plus mileage reimbursement. Travel within the Dallas service area is required, with 1 weekend per month on-call.

• Graduate of an accredited Physical Therapy program.
• Active Physical Therapist license in the state of assignment.
• At least 1 year of home health experience and experience with HCHB.
• Willingness to travel within the assigned coverage area.
• Current BLS certification.
• Valid driver’s license.

Responsibilities

• Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks.
• Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence.
• Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education.
• Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques.
• Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems.
• Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides.
• Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team.
• Participate in case conferences, care coordination activities, and team collaboration.
• Ensure compliance with CMS regulations, infection control standards, and agency policies.
• Manage a mobile caseload efficiently while meeting productivity and documentation expectations.
• Adapt quickly to new environments, workflows, and patient populations as a travel clinician.

Job Title: Physical Therapist - Home Health

Location: Dallas, TX [Coverage Areas: Dallas service area

Schedule: Monday - Friday, 8:00 AM - 4:30 PM

Duration: 6+ Months

Pay Rate: $55.00/hour (W-2) Plus Mileage reimbursement

*Negotiable & Pay offered is based on experience,expertise, credentialing, and education.

Weekly Hour Guarantee: 40 hours

On-Call/Weekend Requirement: 1 weekend per month

Required Qualification/Certification
