Pride Health is actively hiring a Travel/local - Physical Therapist (PT) to join our Home Health team in Tallahassee, FL. Schedule: Monday-Friday, 8:00 AM - 4:30 PM. Location: Tallahassee, FL, 32308. Contract: Local, 14 weeks with possible extension.

Pay: $50.00 to $57.00/hour. Mileage reimbursement: $0.53/mile. Responsibilities include PT evaluations, plans of care, interventions for orthopedic/neurological conditions, patient education, and OASIS documentation.

Qualifications

• Active Physical Therapist license is required.
• Current CPR certification.
• Valid driver’s license with reliable transportation and auto insurance.
• Strong patient assessment and treatment planning skills.

Responsibilities

• Perform comprehensive physical therapy evaluations and determine patient eligibility for therapy services.
• Develop, implement, and revise individualized plans of care based on physician referrals, patient assessments, and clinical goals.
• Provide therapeutic interventions for orthopedic, neurological, and neuromuscular conditions.
• Improve patient mobility, strength, balance, endurance, safety, and functional independence.
• Educate patients and caregivers regarding therapeutic exercises, home safety, fall prevention, and adaptive equipment use.
• Complete accurate and timely clinical documentation, including required OASIS assessments and progress notes.
• Coordinate care with physicians, nurses, caregivers, and interdisciplinary healthcare team members.
• Ensure compliance with Medicare, CMS, agency, and regulatory requirements.

Job Type: Local Contract
Contract Length: 14 Weeks (Possible Extension)

*Pay offered is based on experience,expertise, credentialing, and education.
