Pride Health is seeking a Physical Therapist (Home Health) to support a client facility in Waldorf, MD. Schedule is Monday–Friday, 8:00 AM–4:30 PM, with coverage across Harles, Calvert, and St. Mary’s counties.

This local contract offers a pay range of $56–$60/hour (W-2) plus mileage reimbursement. The role involves travel as a clinician, comprehensive PT evaluations, care plan development, and collaboration with an interprofessional team.

Qualificações

• Active Physical Therapist license.
• Minimum 1 year home health experience; familiarity with HCHB and OASIS is a plus.

Responsabilidades

• Perform comprehensive physical therapy evaluations and follow-up reassessments.
• Develop and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management and functional independence.
• Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education.
• Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques.
• Document evaluations, visit notes, progress reports and discharge summaries in EMR systems.
• Coordinate care with physicians and interdisciplinary team members.
• Communicate changes in patient condition or barriers to progress to the care team.
• Participate in case conferences and care coordination activities.
• Ensure CMS regulations and infection control standards are followed.
• Manage a mobile caseload while meeting productivity and documentation expectations.
• Adapt quickly to new environments and patient populations as a travel clinician.

Pride Health is hiring a Physical Therapist (Home Health) to support our client's healthcare facility in Waldorf, MD.

Job Details

Schedule: Monday-Friday | 8:00 AM-4:30 PM

Location: Waldorf, MD

Coverage Area: Harles, Calvert, St. Mary's counties

Contract Length: 15+ Weeks Contract (Possibility to Extend)

Job Type: Local Contract

Pay Range: $56.00 - $60.00/hour (W-2) Plus Mileage reimbursement

Mileage Reimbursement Rate: $0.52/mile

*Pay offered is based on experience, expertise, credentialing, and education

Duties

• Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks.


• Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence.


• Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education.


• Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques.


• Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems.


• Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides.


• Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team.


• Participate in case conferences, care coordination activities, and team collaboration.


• Ensure compliance with CMS regulations, infection control standards, and agency policies.


• Manage a mobile caseload efficiently while meeting productivity and documentation expectations.


• Adapt quickly to new environments, workflows, and patient populations as a travel clinician.

Requirements

• License(s): Active Physical Therapist license


• Years of experience: At least 1 year of home health experience and experience with HomeCare HomeBase (HCHB) and OASIS is a plus
