Flagstar Rehab is seeking a licensed Physical Therapist (PT) to join our team in Skilled Nursing Facilities. The role offers flexible scheduling, per diem options, and strong support for independent contracting paths.

We require a NYS PT license, BLS, and EMR proficiency, with opportunities to supervise PTAs and deliver high-quality care to patients with diverse diagnoses. Competitive 1099 compensation is provided.

Qualificações

• Valid BLS certification and EMR competence expected.
• Ability to provide efficient evaluations and create treatment plans.

Responsabilidades

• Provide efficient evaluations and develop individualized treatment plans for PTAs to follow under supervision.
• Deliver quality therapy to SNF residents with conditions like stroke, TBI, Parkinson’s, MS, and orthopedic issues.
• Monitor and document patient progress using EMR software.

Active New York Physical Therapist License Required

Compensation:

• 1099: $64/hour

Physical Therapist Requirements and Responsibilities:

• Current NYS PT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
