Flagstar Rehab is seeking Physical Therapists to join Skilled Nursing Facilities with the freedom to choose assignments that fit your schedule and a “choose the way you earn” model.

Requirements: Current NYS PT License and Registration, valid BLS, EMR competence, and eligibility to work in a Nursing Facility. We offer flexible schedules (FT, PT, per diem) and weekly pay with a range of benefit options.

Qualifications

• Current NYS PT License and Registration.
• Valid BLS Certification.
• EMR competence and ability to document patient progress.

Responsabilités

• Provide efficient evaluations and individualized treatment plans for PTAs to follow under supervision.
• Deliver quality therapy services in SNF settings for conditions such as TBI, CVA, stroke, Parkinson’s, MS, cancer, and orthopedic surgery.
• Monitor and document patient progress using EMR software.

Flagstar Rehab is Seeking Physical Therapists (PT) for Skilled Nursing Facilities!

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Requirements and Responsibilities:

• Current NYS PT License and Registration

• Valid BLS Certification

• EMR Competence

• Meets all State and Federal requirements for US employment in a Nursing Facility

• Malpractice/Liability Insurance (if applicable)

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.

• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

• Monitor and document patient progress with the use of innovative EMR software.
