Job summary

Flagstar Rehab is hiring Physical Therapists for state-of-the-art skilled nursing facilities in NYC and surrounding areas. You will perform evaluations, develop PT plans, and supervise PTAs in a fast-paced rehab department.

We offer direct employment or independent contractor options, with pay rates of $46–$48 hourly (W-2) or $57–$60 hourly (1099). Weekly pay and flexible scheduling are available.

Qualifications

• Current NYS PT License and Registration is required.
• CPR Certification is required.
• EMR competence or experience with EMR systems is needed.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/liability insurance (if applicable) may be required.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily and oversee therapy delivery.
• Deliver quality therapy services in a skilled nursing facility setting for conditions such as TBI, stroke, Parkinson’s, MS, cancer, and orthopedic surgeries.
• Monitor and document patient progress using EMR software.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
