Job summary

Flagstar Rehab is seeking licensed Physical Therapists to join our team in skilled nursing facilities across NYC and surrounding areas. You will conduct evaluations, develop individualized treatment plans, supervise PTAs, and educate patients and families to improve physical function and daily living.

We offer flexible employment options: direct employment or as an Independent Contractor. Pay is $46–$48 per hour (W-2) or $57–$60 per hour (1099), with weekly pay, paid orientation, sick leave, and

• Current NYS PT license and registration.
• CPR certification.
• EMR competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:
