Flagstar Rehab is seeking a Physical Therapist (PT) for a skilled nursing facility in the NYC area. You will perform patient evaluations, develop individualized treatment plans, and supervise PTAs while delivering high-quality therapy in a multi-disciplinary SNF team.

This role requires NY licensure, CPR certification, and EMR proficiency, with opportunities for varied shifts and a supportive environment. Flexibility and dedication to patient outcomes are valued.

Qualifications

• Licensed PT in NY with current registration.
• CPR certification as a required credential.
• Experience in skilled nursing facilities is preferred.

Responsibilities

• Perform evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily in the SNF setting.
• Deliver quality therapy services to patients with diverse diagnoses (e.g., stroke, TBI, orthopedic conditions).
• Monitor and document patient progress using EMR.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
