Flagstar Rehab in New Jersey seeks licensed Physical Therapists (PT) to join our SNF teams. You’ll lead efficient evaluations, create individual plans, and supervise PTAs to deliver quality care.

We offer flexible scheduling (FT/PT/Per Diem, weekends, days/evenings), fully paid orientation, paid sick leave, commuter benefits, and weekly direct deposit. ICs may not qualify for all benefits.

Qualifications

• Current NJS PT License and Registration.
• Valid BLS Certification.
• EMR software competence.
• Meets state and federal employment requirements for US nursing facilities.

Responsabilités

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under supervision.
• Provide quality therapy services to SNF residents with varied diagnoses and needs.
• Monitor and document patient progress using EMR software.

Hourly Pay Rate: $43-$60

Pay rate commensurate with experience

Physical Therapist Requirements and Responsibilities:

• Current NJS PT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
