Job summary

Flagstar Rehab is seeking Physical Therapists (PT) to fill roles in Skilled Nursing Facilities. The team emphasizes career flexibility, a “choose the way you earn” model, and support for independent work setups.

Candidates must hold NYS PT license, BLS, and EMR skills, with opportunities across Full-Time, Part-Time, and Per Diem schedules. Pay rates range from $46–$48 per hour (W-2) or $57–$60 per hour (1099), with weekly pay and a comprehensive onboarding process.

Qualifications

• Current NYS PT License and Registration.
• Valid BLS Certification.
• EMR Competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, MS, Cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress with the use of EMR software.
