Job summary

Flagstar Rehab is hiring Physical Therapists (PT) to work in Skilled Nursing Facilities across NYC and surrounding areas. You will evaluate patients, develop treatment plans, and supervise PTAs as part of a multi-disciplinary team.

The role offers direct employment with the agency or Independent Contractor options, with pay rates from $46–$60 hourly (W-2 and 1099 respectively). Weekly pay and flexible schedules are available.

Qualifications

• Current NYS PT License and Registration is required.
• CPR Certification is required.
• EMR competence or familiarity with EMR software is preferred.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting with diverse diagnoses.
• Monitor and document patient progress using EMR software.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state‑of‑the‑art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
