Job summary

Flagstar Rehab is seeking a Physical Therapist (PT) for a Skilled Nursing Facility in NYC and surrounding areas. You will evaluate patients, develop individualized treatment plans, supervise PTAs, and educate patients and families to improve function and cognition.

This role can be direct employment with Flagstar Rehab or independent contractor. Pay rates are $46-$48 per hour (W-2) or $57-$60 per hour (1099); company offers paid orientation, sick leave, commuter benefits, weekly pay, and 24/7

Qualifications

• Active NYS PT license and registration.
• CPR certification.
• Proficient with EMR systems and documentation.

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Deliver quality therapy services in a SNF setting for patients with various conditions.
• Monitor and document patient progress with EMR software.

Physical Therapist (PT) in Skilled Nursing Facility

Now Staffing Physical Therapists for Rehab Department in state-of-the-art SNFs throughout NYC and Surrounding Areas.

Flagstar Rehab PTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Physical Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.

Physical Therapist Requirements:
