Job summary

Creekside Healthcare Center is hiring a Physical Therapist (PT) to join our team in California. We offer a competitive salary range, comprehensive benefits, and a sign-on bonus. Qualifications

• 1 year of clinical experience preferred; new graduates welcome.

Responsibilities

• Evaluates, plans and administers treatment for neuromuscular and musculoskeletal issues.
• Administers tests for muscle strength, ROM, and respiratory/circulatory function.
• Provides direct resident treatment per plan of care and documents accordingly.
• Conducts home assessments to ensure safe discharge.
• Maintains patient confidentiality and adheres to regulations.

Physical Therapist (PT) Duties and Responsibilities within scope of practice and federal guidelines:

Evaluates, plans and administers treatment to patients with problems related to neuromuscular and musculoskeletal systems.

• Administers and interprets tests and measurements for muscle strength, coordination, range of motion, and respiratory and circulatory efficiency.
• Provides direct resident treatment in accordance with an established plan of care..
• Maintains confidentiality of patient records.
• Conducts home assessments simulated or actual, as appropriate, to ensure discharge of residents to a safe environment.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.

Physical Therapist (PT) Qualifications and Skills:

• Bachelor of Science, Master of Science, or Doctorate degree in Physical Therapy.
• One (1) year of clinical experience preferred. New graduates welcomed!
• Current license to practice Physical Therapy issued by the state of California.
