Job summary

Creekside Healthcare Center is seeking a licensed Physical Therapist (PT) to join our supportive team in California. The role offers a salary range of $104,000–$124,000 per year, with per diem/on-call duties and a sign-on bonus.

PTO and referral bonuses are part of the benefits, and we welcome new graduates as well as experienced clinicians to apply.

Qualifications

• One year of clinical experience preferred; new graduates welcomed.
• Currently licensed to practice Physical Therapy in California.
• Bachelor, Master, or Doctorate in Physical Therapy required or equivalent.

Responsibilities

• Evaluates, plans and administers treatment to patients with neuromuscular and musculoskeletal issues.
• Administers and interprets tests and measurements for muscle strength, coordination, range of motion, and respiratory/circulatory function.
• Provides direct resident treatment following an established plan of care.
• Maintains confidentiality of patient records.
• Conducts home assessments to ensure safe discharge environments.
• Completes required forms and documents per company policy and state/federal regulations.

Physical Therapist (PT) Duties and Responsibilities within scope of practice and federal guidelines:

• Evaluates, plans and administers treatment to patients with problems related to neuromuscular and musculoskeletal systems.
• Administers and interprets tests and measurements for muscle strength, coordination, range of motion, and respiratory and circulatory efficiency.
• Provides direct resident treatment in accordance with an established plan of care.
• Maintains confidentiality of patient records.
• Conducts home assessments simulated or actual, as appropriate, to ensure discharge of residents to a safe environment.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.

Physical Therapist (PT) Qualifications and Skills:

• Bachelor of Science, Master of Science, or Doctorate degree in Physical Therapy.
• One (1) year of clinical experience preferred. New graduates welcomed!
• Current license to practice Physical Therapy issued by the state of California.
