Job summary

Amergis is seeking a Physical Therapist for a travel assignment in Sacramento, CA. You will perform patient evaluations and provide therapy services in accordance with the plan of care and physician orders.

The role requires current PT licensure, graduation from an accredited PT program, and a preference for at least one year of experience. Competitive weekly pay and comprehensive benefits are offered.

• One (1) year of prior professional Physical Therapy experience preferred
• Current CPR if applicable
• TB Questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age

Responsibilities

• Perform patient evaluations and document therapy services per the plan of care
• Adhere to physician orders and facility policies
• Maintain accurate treatment records and progress notes

The Physical Therapist is responsible for performing patientevaluations, as well as providing and documenting therapy services inaccordance with the plan of care developed for each individual patient andphysician’s orders when applicable.

Minimum Requirements
