Glendale Transitional Care Center is hiring a Physical Therapist (PT) to evaluate and treat residents with neuromuscular and musculoskeletal conditions in a supportive environment.

The role offers a salary of $104,000–$124,000 per year with per diem/on-call frequency and a sign-on bonus. California licensure is required; new graduates welcomed and encouraged to apply.

Qualifications

• Must hold a current California Physical Therapy license.
• Bachelor, Master, or Doctorate in Physical Therapy.
• One year clinical experience preferred; new graduates welcome.

Responsibilities

• Evaluates, plans and administers treatment for neuromuscular and musculoskeletal issues.
• Administers and interprets tests for strength, ROM, and respiratory/circulatory efficiency.
• Provides direct resident care per established plan of care.
• Maintains confidentiality of patient records.
• Conducts home assessments to support safe discharge from facility.
• Completes forms and documents per company policy and regulations.

Physical Therapist (PT) Duties and Responsibilities within scope of practice and federal guidelines:

• Evaluates, plans and administers treatment to patients with problems related to neuromuscular and musculoskeletal systems.
• Administers and interprets tests and measurements for muscle strength, coordination, range of motion, and respiratory and circulatory efficiency.
• Provides direct resident treatment in accordance with an established plan of care.
• Maintains confidentiality of patient records.
• Conducts home assessments simulated or actual, as appropriate, to ensure discharge of residents to a safe environment.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.

Physical Therapist (PT) Qualifications and Skills:

• Bachelor of Science, Master of Science, or Doctorate degree in Physical Therapy.
• One (1) year of clinical experience preferred. New graduates welcomed!
• Current license to practice Physical Therapy issued by the state of California.
