Job summary

Glendale Transitional Care Center is hiring a Physical Therapist (PT) to evaluate and treat residents with neuromuscular and musculoskeletal conditions. The role includes planning and administering therapy per a care plan, performing assessments, and ensuring proper documentation.

A California PT license is required, and new graduates are welcome. The position offers a sign-on bonus, benefits, and a salary range of $99,840 to $124,800 annually for full or part-time work, with relocation and PTO

Qualifications

• Bachelor/Master/Doctorate in Physical Therapy from an accredited program.
• 1 year of clinical experience preferred; new graduates welcomed.
• Current license to practice Physical Therapy issued by the state of California.

Responsibilities

• Evaluates, plans and administers treatment to patients with neuromuscular and musculoskeletal issues.
• Administers tests and measures for strength, ROM, coordination, and respiratory/circulatory function.
• Provides direct resident treatment per established plan of care and maintains confidentiality.
• Conducts home assessments to support safe discharge and documents according to policy.

Physical Therapist (PT) Duties and Responsibilities within scope of practice and federal guidelines:

Evaluates, plans and administers treatment to patients with problems related to neuromuscular and musculoskeletal systems.

• Administers and interprets tests and measurements for muscle strength, coordination, range of motion, and respiratory and circulatory efficiency.
• Provides direct resident treatment in accordance with an established plan of care..
• Maintains confidentiality of patient records.
• Conducts home assessments simulated or actual, as appropriate, to ensure discharge of residents to a safe environment.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.

Physical Therapist (PT) Qualifications and Skills:

• Bachelor of Science, Master of Science, or Doctorate degree in Physical Therapy.
• One (1) year of clinical experience preferred. New graduates welcomed!
• Current license to practice Physical Therapy issued by the state of California.
