Job summary

Advocate Health Care is seeking a licensed Physical Therapist in Illinois to provide comprehensive patient evaluations, develop and adjust treatment plans, and ensure safe, effective care across musculoskeletal and related areas.

You will discharge patients with home programs, coordinate with physicians and staff, and document progress while meeting department standards and productivity goals.

Qualifications

• Excellent communication, interpersonal, and organizational skills.
• Intermediate computer skills required.
• Demonstrated organization, decision-making and problem-solving abilities.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary and/or integumentary evaluations.
• Establishes a plan of care and implements treatment based on evaluations and goals.
• Monitors outcomes and adjusts plans; communicates with physicians and teammates.
• Plans for discharge and provides home programs and patient education.
• Documents all care per policy and maintains productivity standards.
• Cleans up after treatment and follows safety/infection control measures.
• May supervise PT aides/assistants and students as required.
• Participates in performance improvement and staff orientation.
