Advocate Health Care in Chicago is seeking a licensed Physical Therapist to evaluate, treat, and discharge patients across musculoskeletal, neuromuscular, cardiopulmonary, and integumentary systems. You will collaborate with physicians, plan care, and document progress.

This role includes enabling home programs, ensuring safety, and contributing to performance improvement. A doctorate in physical therapy and state licensure are required, with BLS certification within 90 days.

Qualificações

• Doctorate degree in Physical Therapy from an accredited school.
• Maintains continuing education requirements as mandated by applicable state standards.
• Licensed Physical Therapist in the state you work; BLS for Healthcare Providers within 90 days.

Responsabilidades

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment, monitors outcomes, and adjusts as needed.
• Prepares discharge plans including home programs and patient/family education; coordinates with caregivers.
• Completes documentation per policy including evaluations, progress notes, and discharge summaries; handles billing and scheduling per guidelines.
• Performs patient handling duties, utilizes modalities and equipment, and ensures safety and infection control.
• Participates in performance improvement activities and collaborates with site leadership on coverage needs.
