Job summary

Advocate Health seeks a licensed Physical Therapist to evaluate patients, develop and implement comprehensive treatment plans, and monitor progress. The role requires data gathering from medical records, physicians, patients, and families, with ongoing reassessment to adapt plans of care.

Documentation of evaluations, progress notes, and discharge plans is essential. Supervision of PTA's and Rehab Tech/Aide is included as required for licensure.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Applicable state license for Physical Therapy required.
• BLS required per policy guidelines.

Responsibilities

• Evaluate patients and select appropriate assessment techniques to establish care plans.
• Gather data from medical records, physician reports, patient and family.
• Establish functional goals and provide evidence-based interventions with ongoing reassessment.
• Document evaluations, treatments and progress notes, discharge plans and charges.
• Communicate progress to physicians, referral sources and clinical staff; coordinate schedules and discharge planning.
• Supervise PTA's and Rehab Tech/Aide as required for licensure.
