Advocate Health in Illinois is seeking a Physical Therapist to evaluate patients, develop and implement treatment plans, monitor progress, and adjust care as needed. The role includes discharge planning, patient education, and collaboration with physicians and teammates.

The position requires a Doctorate in Physical Therapy and an active PT license, with strong communication and organizational skills, and the ability to manage caseload and complete accurate documentation.

Formación

• Doctorate in Physical Therapy from an accredited program and active state licensure.
• Excellent communication and interpersonal skills with patients and team members.
• Strong written and verbal communication; proficient with standard computer systems.
• Ability to plan, implement, and adjust care plans based on patient progress.
• Ability to manage caseload, complete documentation per policy, and meet productivity standards.

Responsabilidades

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary and/or integumentary evaluations.
• Establishes and implements plan of care and monitors outcomes; adjusts plan as needed.
• Plans discharge and provides home programs, patient education, and referrals.
• Documents assessments, progress notes, discharge summaries, and billing- related records.
• Operates with safety and infection control; participates in quality improvement activities.
• May supervise assistants and rehab staff and provides orientation and feedback.
• Pursues ongoing professional growth and participates in department education.
