Job summary

Continuum Therapy Partners is seeking a part-time Physical Therapist for daytime hours at Elmwood Health & Rehab Center in West Carrollton, OH. The role requires a PT license and NPTE, with 1 year of experience preferred, to evaluate, plan, and implement individualized treatment plans.

You will document progress, adjust plans based on outcomes, and educate patients and families on home exercise programs and injury prevention.

Qualifications

• Degree from an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate).
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE).
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
