Job summary

Continuum Therapy Partners is seeking a Physical Therapist to join our team at Heritage Hall Rich Creek, Rich Creek, VA. You will evaluate patients, develop and implement treatment plans, and coordinate with other disciplines to optimize outcomes.

The role emphasizes documenting progress, adjusting plans based on results, and educating patients and families on home exercise programs and injury prevention. A state license and NPTE completion are required.

Qualifications

• Degree from an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate).
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.

Physical Therapist Requirements & Responsibilities:
