Continuum Therapy Partners is seeking a Physical Therapist to join our team at Windsor Health & Rehabilitation in Starke, FL. A Doctor of Physical Therapy is required, with an active state license and NPTE.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
